Nuvance Health: Diabetes Prevention Program at Northern Dutchess Hospital
Are you at risk for diabetes? Take charge of your health and make a change.
Northern Dutchess Hospital’s National Diabetes Prevention Program is part of the national program led by the Centers for Disease Control and Prevention (CDC). Our program (DPP lifestyle change program) runs for 1 year. In the first 6 months we will meet weekly for an hour and learn about;
- Healthy eating without giving up all the foods they love.
- Adding physical activity into their schedules.
- Dealing with stress.
- Coping with challenges that can derail their progress.
- Getting back on track if they stray from their plan.
Our program has been running continuously successfully for over 10 years. The group setting is a constant support system, one in which you will feel comfortable to share ideas, struggles, goals and most of all successes. All of this is at no cost to you. All you have to do is sign up and commit.

When: Mondays, starting on March 16, 2026
Time: 4:30pm or 6pm (choose one)
Where: Northern Dutchess Hospital Cafeteria Conference Room 6511 Springbrook Avenue, Rhinebeck
(Yearlong program with 16 weekly meetings and bi-monthly follow-up meetings)
*We meet on Teams and in person every week.
